At Reliant, we take identity theft seriously and understand how challenging it can be. If you suspect someone has fraudulently used your identity to start a service with Reliant, we’re here to guide you through the next steps.

Here’s what to do to protect your identity:

  1. Chat or call us immediately at 1-866-222-710 — we’re available 24/7.
  2. Check to see if your identity was used to open an account.
  3. Complete a notarized  ID Theft Affidavit.
  4. File a police report, and request a complete copy.
  5. Send all required documents to Reliant.

Documents needed to dispute an account

To help us investigate, please provide the following:

  • Copies of 2 different utility bills or formal correspondence in your name from other service providers.
  • A notarized  ID Theft Affidavit, on which you have indicated that you are willing to press charges.
  • A complete copy of the police report.
  • A valid government-issued photo ID card (for example, your driver's license, state-issued ID card or passport).
  • If required, a clear copy of your Social Security card to verify your identity.

What is an ID Theft Affidavit?

An ID Theft Affidavit is a standardized document designed to help companies like Reliant efficiently handle identity theft claims.

Important things you need to know:

  1. Provide accurate and complete information to avoid delays
  2. Sign the affidavit in the presence of a witness or notary
  3. Opt in to press charges for the investigation to move forward
  4. If you cannot complete the affidavit yourself, a legal guardian or someone with power of attorney can fill it out for you
